Bring two passport photographs, proof of U.S. citizenship, and a valid form of photo identification, such as a driver's license. Application forms are available at the Library or from the U.S. Department of State's Website at http://travel.state.gov. Passport photos can be taken at time of application by Library staff. Cost is 2 photos for $10.00.
